PLEASE READ ALL PROCEDURES BEFORE COMPLETING THE RESERVATION REQUEST.

1.  Complete the reservation form completely.  Once approved, your request will be placed on the Church Van Calendar.  Updated calendars are available on the church website.

 

2.  Before use, a copy of the driver’s license of the individual driving the van (min. age: 25 years old) needs to be submitted to the church office (no exceptions).  
 
3.  One week’s notice should be given prior to date needed.
 
4.  The keys will be ready for pickup during church office hours (9:00 am to 5:00 pm – Mon. – Thurs.) – (9:00 am to Noon – Fri.), unless other arrangements have been made with the transportation contact.
 
THE DRIVER IS RESPONSIBLE FOR:
 
  • Insuring all passengers are wearing a seatbelt.
  • Completing the trip mileage log (located in the van).
  • Making pre- and post-trip checks of oil and fuel levels.
  • Van height is 10 feet.  Please be mindful of this to avoid damage to rooftop.
  • Reporting any needed repairs on the trip report (located in the van).
  • Returning the van with a full tank of fuel.
  • Fuel receipts for reimbursement shall be turned into the financial secretary and funds will be taken from the ministry that used the van(s).
 
FINAL CHECKLIST:
 
  • Insure all windows and doors are secured.
  • Remove all trash and personal items from the van(s).  Note:  any remaining items will be discarded.
  • Return key, fuel receipts for reimbursement to the church office.
 
5.  Accidents involving the church van(s) should be reported to the proper authorities and a copy of the police report given to the church office when returning the key.
 
6.  Any expenditures for major repairs should be authorized by the church office.
